4-Week 30-Minute Music Lesson Agreement

This agreement is between Mark S. Schwartz, doing business as Super Key Music Studios (“Studio”), and ______________________________ (“Customer”).
Tuition
Tuition is $100.00 for four (4) 30-minute __________________ lessons for the student named above.
Lessons are scheduled for ______________________________ at ______________________________, beginning ______________________________.
To continue for another four (4) lessons and keep the same lesson time, the next tuition payment is due on or before the final lesson of the current four-lesson package. The Customer is responsible for keeping track of package renewals and payment dates.
Make checks payable to Super Key Music Studios or Mark Schwartz. Sorry, no out of town/out of state checks and no refunds.
Cancellations and Make-Up Lessons
If the Studio cancels a lesson, the Studio and Customer will arrange a make-up lesson at a mutually convenient time. A Studio cancellation does not change the tuition due date.
Lessons missed or cancelled by the Customer are ordinarily not made up and do not delay tuition payments.]
Regular Student Make-Up Privilege
A student may earn one (1) make-up lesson after completing twelve (12) consecutive weeks of lessons, provided that all of the following requirements are met:
The Customer continues by purchasing the next four-lesson package on or before the final lesson in the current package.
The Studio receives notice of the cancellation at least one (1) hour before the scheduled lesson time.
The student’s tuition account is current.
The make-up lesson is scheduled and attended on or before the final lesson in the current paid package.
The make-up privilege is intended for regularly continuing students and does not apply when there is a lapse in enrollment or payment.

By signing the agreement provided by the Studio, the Customer confirms that they have read, understand, and agree to these terms.
