This agreement is entered into between Mark S Schwartz, dba Super Key Music Studios ("Owner") and ("Name of Customer").

• Tuition

  • Tuition is $200.00 for Name of Student (4) one-hour (Course of Study) lessons. The lessons will be at Time of Day am/pm on Day of Week starting Name of Month, 2015.

  • If you want to continue another group of (4) one-hour lessons and guarantee the same time slot. Tuition is due on or before the last lesson in your pay-period. (It is also the responsibility of the student/ customer to keep track of his/her payments.

Make checks payable to Super Key Music Studios and/ or Mark Schwartz. Sorry, no out of town/out of state checks and no refunds.

• Cancellations / Make up Lessons

  • Any lessons cancelled by your instructor will be made up at both the student and instructors convenience. The canceled lesson(s) does not permit late or incomplete payment of Tuition.

  • Under the standard (4) week agreement any lesson(s) cancelled or missed by the student will not be made up.

  • Missed lessons do not constitute putting off payment of Tuition. Any missed lesson(s) are a loss under the (4) week plan unless you qualify for the Regular Student Privilege. One Make-Up-Lesson after every 12 consecutive weeks.

  • For Make-Up-Lesson privileges, ask us about our 8 week Advanced Payment Plan.

  • In order to receive Make-Up-Lesson privileges, we must be notified of cancellations at least an hour before your designated lesson time.

  • Make-Up-Lessons must be attended on or before the last lesson in your pay-period.

I understand and agree to the policies mentioned in this agreement. I also realize this agreement is in affect for the duration of your lessons at Super Key Music Studios, unless both parties agree to amend the agreement.
