This agreement is entered into between Mark S. Schwartz, dba Super Key Music
Studios (“Owner”), and Name of Customer (“Customer”).
Tuition:
Make checks out to Mark Schwartz. Sorry, no out of town/out of state checks and no
refunds.
Cancellations/Make-Up Lessons:
I understand and agree to the policies of Super Key Music Studios mentioned in this
agreement. I also realize that this agreement is in affect for the duration of your
lessons at Super Key Music Studios, unless both parties agree to amend the
agreement.

Tuition:
- Tuition is $142.50 (includes a 5% discount) for Name of Student eight half-hour
Name of Course lessons. See chart for Advanced Payment Plans. The
lessons will be at Time of Day am/pm on Day of Week day starting Name of
Month, 2010. - If you want to continue lessons and guarantee your time slot, tuition is due on or
before the last lesson of your pay period. (It is also the responsibility of the
student/customer to keep track of his/her payments).

Cancellations/Make-Up Lessons:
- Any lessons cancelled by your instructor will be made up at both the student
and teacher’s convenience. The cancelled lesson(s) does not permit late or
incomplete payment of tuition. - Under the standard four-week agreement, any lessons canceled or missed by
the student will not be made up. (See following point for Regular Student
Privilege.) - Missing lessons does not constitute putting off payment of tuition. Any missed
lessons are a loss under the four-week pay plan, unless you qualify for the
Regular Student Privilege. - For make-up lesson privileges, ask us about our Advanced Payment Plans.
(See chart below) - In order to receive make-up lesson privileges, we must be notified of
cancellations at least an hour before the lesson to be missed. - Make-up lessons must be attended on and/or before the last lesson
